A different angle to this great thread..
Dunno if this has been observed elsewhere, but a German former pro 
guitarplayer I know had to give up any band efforts due to tinnitus.
He always used IEM and never at high levels. Dunno which brand/model.

He got the tinnitus on one of his ears from wearing his custom molded 
EIM system all through a seven hour monster gig.
Doctors told him it was because the custom molds had such a perfect fit 
that he'd been lacking differential pressure (re)alignment.

Of cause I'm not warning against EIM's, but even though this may be a 
special case, I do find it worth the while noting that such may happen.
